Free & Reduced Lunch Form:
This form allows The Greater Dayton School to determine your students eligibility for Free or Reduced lunch. The data shared in this form will only be used for this purpose. Your child will not be charged for meals they receive regardless of the determination, however, this data allows our school to continue our programing.
Participation in Activities Waiver:
This Release allows your students to participate in extracurricular and Off-Campus activities.
Medical Waiver:
This Release allows your students to receive medical treatment through Dayton Children’s Hospital, Primary Health Solutions and Onesight Vision Clinic so that we can provide your students well-child visit, routine dental cleanings and eye-vision screenings.
Technology & Network Agreement:
This acknowledgement should be signed by both the student and parent/guardian to acknowledge that the student agrees to be respectful and act responsibly when using technology at the Greater Dayton School.
Therapy History Form:
Please complete if your child has received speech, occupational, mental health or behavioral therapy.

Kindergarten students and older:
Medicine Authorization:
This form allows GDS staff to administer basic over-the-counter medications to students in grades K-4, such as Pepto Bismol, Tylenol, etc. Students cannot carry medication on their person without specific written permission from the prescribing physician filed in The Greater Dayton School health office.
EdChoice Income Verification Form:
Please complete this form if ONLY requested by the Greater Dayton School.
